Paper-based Student Registration

Step 1

Download the forms linked below and print them out - we have both Spanish and English available

Step 2

Distribute forms to students and request a return date that is no less than one week before your club’s scheduled start date.

**Late submissions cannot guarantee students will be added to the system on time for the first session of the club

Step 3

Compile all submitted forms and scan them. Send all scans to Bold Idea’s Program Coordinator

Step 4

Within a week of sending Bold Idea the forms, we will follow up a spreadsheet of all student log-ins and instructions for how to get them set up on the first day of the club.
